HAVE YOU LOST THE FAMILY BOOK?

If you have lost your Family Book you must go to the Civil Registry to request a duplicate.
It must be the same office where they gave you the original document.
To get a duplicate of the Family Book you will need the following documents:
-DNI of the two spouses.
-DNI or birth certificate of each of the children.
